We visit you at home
Operating from our office in Cobram, our Home Care service provides assistance to clients across the Moira and Berrigan shires. We offer an extensive range of home and community-based care services to suit your lifestyle and circumstances. Whether you need some basic help around the house on a temporary or ongoing basis, we have the flexibility to assist you however you choose.
What does ‘Support at Home’ Provide?
Under the Support at Home program, you have access to a defined list of services to support you to live at home for as long as possible.
This can include support for:
- clinical supports, such as nursing care, occupational therapy, and podiatry
- independence, such as help with showering, taking medications, transport, and respite care
- everyday living, such as cleaning, gardening, shopping, or meal preparation.
It also includes services that support culturally safe care, such as help to join in cultural activities and access to Aboriginal and Torres Strait Islander health practitioners and health workers.
The support is provided through a Support at Home – a coordinated mix of services that can include:
- help with household tasks
- equipment (such as walking frames)
- minor home modifications
- personal care
- clinical care such as nursing, allied health and physiotherapy services.

Level 1: Basic support for those who need minimal assistance

Level 2: Moderate support for individuals requiring some help with daily activities

Level 3: Increased support for those needing more comprehensive assistance

Level 4: High-level support for individuals with significant care needs

Level 5: Specialized support for those with complex health conditions

Level 6: Intensive support for individuals requiring extensive care

Level 7: Comprehensive support for those with very high care needs
- Level 8: Maximum support for individuals needing full-time care and assistance
What does Support at Home cost?
- Hourly Charges – Each registered provider sets pricing based on the total costs of delivering a service. At CRC our hourly charges range from $95.00 for domestic assistance up to $250.00 for specialist Allied Health therapy at home. Surcharges apply for evenings, weekends and public holidays.
- Funding Level – The Support at Home program has replaced the Home Care Packages Program, and costs depend on your assessed level, and income and assets as assessed by Services Australia.
- Government Contribution – The Australian Government contributes to the cost of care, but you may also be asked to contribute based on your financial situation

Who qualifies for Support at Home
If you’re approved for Support at Home, you will receive a letter advising you what services and funding you’re approved for. This includes any approvals for ongoing or short-term care depending on your assessed needs.
When your funding comes through, you can then choose a service provider that is right for you. Services are provided by a variety of organisations across Australia.
Your chosen provider will discuss with you the mix of services to meet your assessed needs and budget. You can request to change this mix at any time. You can also ask for a reassessment if your needs change at a later point.
What is a Registered Provider?
A registered provider is an organisation registered with the Australian Government, assessed for quality and safety, and authorised to deliver government-subsidised in-home aged care, managing funds and services (like personal care, domestic help, or allied health) to support older Australians’ independence using a wellness-focused approach.
IMPORTANT: If you or your loved one is unhappy with the quality of their care or approved provider, you can register a request to change providers through the My Aged Care government website at any time and choose a new provider.
